Camden Rehabilitation Services

A multi-disciplinary team consisting of Occupational therapists, Physiotherapists, a Speech and Language therapist, Dietician, Psychologists and Rehabilitation Assistants offer the following services to Camden patients in their
own homes or community settings:

  • Assessment, retraining and practice of self care and domestic tasks
  • Prevention of hospital admission through rehabilitation and equipment provision
  • Falls intervention when clients are falling or at risk of falls.
  • Resolving problems relating to difficulty with transfers, mobility or support systems
  • Mobility assessment and practice indoors and outdoors
  • Acute musculoskeletal pain managementAssessment and intervention for psychological barriers to rehabilitation
  • Home exercise programme
  • Transfer training or upper limb splinting
  • Replacement of essential independence equipment or walking aids.

 

  • Treatment of dysphagia / dysphasia
  • Treatment of communication difficulties
  • Assistance with reintegration into community life
  • Post hospital discharge, return to independence
  • Assessment and treatment of loss of function or deterioration
  • Rehabilitation management of long term conditions
  • Review of functional ability
  • Assessment and intervention for minor environmental barriers to independence
  • Health promotion
  • Carer advice and support
  • Cognitive assessments and interventions
  • Nutritional advice


  • Patients must consent to the referral and have rehabilitation potential to benefit from therapy input.
  • Patients with significant cognitive problems may not be appropriate.

Falls Service

Provides assessments, support and rehabilitation for people who have had a fall.  Therapists visit patients at home and complete a falls assessment. Following this the patient may receive therapy and equipment provision in their home, be referred to a Medical Clinic or be referred to the 9 week Staying Steady exercise programme.

Stroke Early Supported Discharge Service

Aims to support stroke survivors through facilitating early hospital discharge and empower through providing education and intensive therapy to improve personal safety, independence and enhance skills for self management to improve quality of life. Overall aim is to enable survivors to return home and participate in their lives at the earliest opportunity.

Early Supported Discharge Service

Aims to support clients through facilitating early hospital discharge and empower them through providing therapy to improve personal safety, independence and enhance skills for self management to improve quality of life following hospital admission. Stroke Coordination and Navigation

Responsible for co-ordination and navigation, by monitoring and coordinating progress through the local stroke pathway, especially the minority who do not follow clear pathways. Also responsible for 6 week, 6 month reviews and annual stroke specific reviews.

Neuro service

Provides multi-disciplinary therapy for clients with a neurological condition who are over 18 years of age and would benefit from rehabilitation.  The team can provide up to 12 weeks of therapy, with the aim of supporting or improving function, sessions can take place in the community or in clients’ homes.

Rapid Response Service

Urgent response service aiming to prevent unnecessary hospital admissions for acutely unwell patients by assessing and supporting them in their homes.

Carelink

Provides short-term support and enabling care in clients’ homes following a hospital stay or period of ill health in the community.
Carelink also provides support to palliative care clients who are at the end of their life.

Intermediate Care Beds at Ingestre Road

Rehabilitation beds based at Ingestre Road Residential Care Home setting, providing a period of rehabilitation for up to 6-weeks for residents of Camden aged 65 years and over. Medical cover is provided by a local GP Practice and nursing needs met by Camden Adult Community Nurses. Referrals are welcome from GPs and other community services.

COPD Service

The team can assist with diagnosis of COPD & Asthma, prescribe, review medication and condition, educate and support patients with self management and enrol patients on to their Pulmonary Rehabilitation programme, which runs at St Pancras Hospital & Peckwater Centre.

 

Self-care & Education

Pulmonary Rehabilitation Programme - 6 week course for people with chronic obstructive pulmonary disease. Patients can self-refer on 020 3317 5355.  
Physical activity programmes for people with COPD are available from the Active Health Team. See the Exercise Referral section for further information.
