Community Nursing

Community Nurses provide care for patients at home including:

 

• Acute and Chronic wound care, including leg ulcer care
• Elimination and continence care
• Enabling and education in the management of chronic disease
• Support and nursing care for the end of life care needs
• NHS Continuing Care.

Different types of staff provide these services and our teams include:

• Team Coordinator
• Community Matron
• Community Staff Nurse
• Assistant Practitioner
• Trainee Assistant Practitioner
• Health Care Assistant
• Phlebotomist
• Clerical Assistant

Other services include:

• Wound Care clinics - assessment and ongoing management for patients with wounds such as leg ulcers.
• Chronic Disease Management clinics for heart failure, coronary heart disease and respiratory care.

The service is also able to access support from other specialist services such as palliative care, HIV and tissue viability.

The service operates from 0830 to 1000 every day, with the Over Night Nursing Service providing night cover
