About the Service

Camden REACH provides a wide range of rehabilitation and support services.  These services are designed to help you remain independent at home or in a location of your choice within the borough of Camden.

Camden REACH aims to help you in areas that are important to you and which you currently find difficult to manage. 

Many of our services are aimed at people who have difficulty accessing outpatient services and who find it easier to be seen in their own home.

If we think we can help you Camden REACH will see you within a certain timeframe depending on your difficulties and the urgency in which you need to be seen.

We aim to work with you, your family and carers to help you feel confident in the tasks that you currently find difficult. 

If you feel you need extra support we will discuss this with you and can provide you with the options available to you.

Camden REACH can be involved with your care and rehabilitation if you have recently returned from hospital, suffered an injury, have a new or long-term illness or if you have had a fall.  All of these may prevent you from getting on with your day to day routine and you may require help to regain your independence or adapt to your new situation.

• Rehabilitation Assistants

There are many areas in which we can help:

Mobility
• Walking aids for indoors and/or outdoors depending on your needs.
• Exercises to help you improve your strength and mobility.
• Advice to help improve your balance and confidence

Promoting Independence
• Meal preparation, eating and drinking.
• Managing eating and swallowing difficulties.
• Washing, dressing, using the bath and toilet.
• Accessing community activities.
• Enable you to communicate to the best of your ability

Reducing Risk of Falls
• Advice to make your home environment safer for you.
• Assessment to see if you are suitable to attend our falls group programme
• Medical assessment to look at why you are falling.

Preventing Hospital Admissions
• Help prevent you from going into hospital due to a chronic illness e.g. a sudden decline in your health or long -term breathing difficulties.

Post Hospital Stay
• Further support and rehabilitation, if required, at home.

Short-term Inpatient Rehabilitation
• We can provide up to 6 weeks of rehabilitation to help you improve your walking and every day activities so that you are safe to return home. 

Respiratory Team
• If you have a diagnosis of asthma or COPD (acute bronchitis/emphysema) this team can provide support and advice to help manage your condition.

Psychology
• Help you to deal with and support you through new changes related to your new circumstances and rehabilitation needs.

You can discuss a referral to our team by calling the main number 0845 900 0684. You can also ask a family member, friend, or health care professional such as your GP, social worker or nurse to call on your behalf.
